AI Vendor Intake — Four Tiers, Three Gates, One Form

This form is the single entry point for every AI vendor entering the organization. It serves Step 2 (Classify the Vendor Universe — Four Tiers, No More, No Fewer) and Step 3 (Architect the Intake Gate — Where Sovereignty Meets Procurement Law) of the AI Vendor Governance Architecture. Procurement officers complete Sections 1–3 at intake. The governance lead completes Section 4 at each gate checkpoint. A 'No' at any active gate activates stop-procurement authority — no RFP, no contract, no deployment proceeds without clearance.
